Parents registering your child for Youth Program football or cheering, please read the instruction below, then go to registration page.

REGISTRATION FORM:
must be fully completed. Please take the time to read and complete the entire form. There are two (2)  pages for registration, first page is to be filled out and returned to MAYFC, to register your child.  The second page of this form, contains a brief description of the league and hopefully will answer some basic questions.  This should be reviewed prior to sending in the registration form.

FUND RAISER OPT-OUT INSTRUCTIONS:
This decision must be made at the time of registration.  This option allows the parent to opt-out of participating in our league fundraiser.  Circle Y to opt-out,  initial and include the $30.00 opt-out fee along with your registration fee.  Circle N to agree to participate in fundraiser and initial. Failure to circle either choice will indicate your intention to participate in the fundraiser.

MEDICAL RELEASE TO PARTICIPATE IN PROGRAM
You must list any medical or physical conditions your child has that our coaches or squad leaders should be aware of.  In the best interest of your child and our coaches please include as much information as possible.

MAYFC is an entirely volunteer organization. It is registered as a Non Profit Organization with the State of Maine.  There are several opportunities to volunteer within this organization such as:

• Team Mom
• Assistant Coaches
• Concession Stand Volunteers
• Referees
• Chain Gang
• Board of Directors

 Most importantly our volunteers need to be available on games days.
